MDPH application documents: what to send, and the mistakes that delay everything

Updated · 7 min read · Written by L'équipe DossierMalin

The short answer

A complete MDPH application contains the CERFA 15692 application form, filled in, dated and signed, the original medical certificate filled in by your doctor, an ID and a proof of address. Never forget part E, which sums up everything you ask for.

What an MDPH application contains

The application is made of two official forms and a few supporting documents. According to Mon parcours handicap, the required documents are:

  • the application form (CERFA 15692), filled in, dated and signed;
  • the medical certificate (CERFA 15695), original, dated less than 12 months ago, filled in by your GP or a specialist;
  • a copy of an ID, front and back, and your legal representative’s if relevant;
  • a proof of address; if someone houses you, their proof of address and a signed statement from them;
  • the court decision if you are under legal protection.

Watch the validity period. The 2019 printed form still mentions a certificate “less than 6 months” old. The official Mon parcours handicap site, updated in June 2026, says 12 months. If in doubt, ask your MDPH.

Medical and social assessments and hospital reports are optional, but they help the team that assesses your situation a lot. For the whole process, from sending the application to the decision, see the MDPH explained.

Which parts of the form to fill in

The form is about twenty pages long, in six parts. You do not have to fill in everything:

Part Content Who fills it in
A Your identity Everyone
B Your daily life, needs and wishes Required, except an unchanged renewal
C School or student life Children, teenagers and students
D Work If your request is about work
E The rights and benefits you ask for Everyone, even though the form presents it as optional
F The family carer Optional

If you only ask to renew your rights as they are, because nothing has changed, the form asks you to fill in parts A and E. You can still fill in the rest.

The most common mistakes

The practical guide from the Centre Ressources Autisme Limousin lists the mistakes it sees most often. They apply to every application:

  1. A medical certificate on plain paper. It must be written on the official form: a certificate on anything else is not accepted.
  2. Forgetting the identity, the date or the signature. An incomplete application cannot be processed.
  3. Forgetting part E. The CRA guide treats it as required: it sums up all your requests, including school ones. Always fill it in.
  4. Asking for a support worker at school in part B. That request goes in part C.
  5. Not briefing the doctor. Tell them whether it is a first certificate or a renewal, and bring your reports.

One more mistake is very common in part B: ticking a need only when you cannot do the thing at all. A need also exists when someone has to remind you, guide you or stay with you. Our part B guide explains this.

How long it takes

The form announces an answer in about 4 months. In 2023, the average processing time was 4.7 months, for nearly 5 million requests filed in the year (CNSA). If your situation is urgent, the form lets you ask for fast processing.

Where to send the application

You can post it to, or hand it in at, the MDPH of your département. Many MDPHs also offer the “MDPH en ligne” online service, where you fill in the form and attach the documents; it is not available everywhere yet. Always keep a full copy of your application.
